Fluid charts are easy. Basically all it is is looking at fluid intake and fluid output and seeing if there is a difference.
    Good luck

In my Trust its a yellow a4 page.

On the left half of the page it is what has been drank time + amount
    Example:
Time: Drink: Total(ML): Total so far(ML):
    0900 Tea 250 250
    1030 Water 100 350

then on the right side it has the output I cant remember exactly how that says but its sort of the same
Time: Amount: (Empty space where we write a comment)
1100 100ml Chamber and Catheter emptied
