That's a bit confusing - are you sure that bit of the form is for you to fill out?

I think that usually, the 'seen' part refers to when the potential employer saw your work permit, and the 'date' part to when the work permit was received, so that part of the form is for them to fill out....does it say anything above that part?

That's why it's confusing it's next to my nationality and marital status in my section of the form. I think i'll just send it off and hope.

Maybe you should just fill in the date you received your work permit...
