Clearing after accepting an unconditional offer

Did this happen before or after results day?
It can happen at either point. When a firm choice is unconditional the only option available to change that decision is for the university to place the applicant into clearing.
However an applicant who does not yet have their results cannot apply for places formally through clearing (they can make enquiries and do research but it isn't possible to apply through clearing without holding your results).
