1) No. It has to be on the UCAS form that you were at uni, but shouldn't cause any problems for you. If you want to explain why, get it in your reference; you want your PS to focus on why the new course is good, not why the old course was bad.

2) I'd go back to your school or college - you have a tutor at uni, but I'd argue they don't know enough about you yet to write one.

3) Sounds like an acceptable reason to me, as long as you don't give the impression you've picked this course as any vocation and persuade them that it's something you want to do.
