    Who should confirm my predicted grades and how ?

    One, but your reference can be written by more than one person. The referee can take comments from other people and include them in the reference.

    Pick the referee who knows you better or preferably you go to school with.

    Generally your referee should be a teacher. They should know you in an official capacity, not as a friend.

    Generally they will say something like "Grades AAB from three A-levels". They could specify subjects, such as "Grades AAB from three A-leveels, including Maths" or "Grade A at Maths A-level and grades AB from two A-levels". It's hugely unlikely that they will make an offer based on five grades, if you're applying to very competitive places then a four grade offer is possible.
