Does the form not have a space where the referee fills in their relationship to you?

If anyone wants to check it then they will and they'll find nothing, don't worry about it.

As long as you haven't got anything to worry about, it will be fine! They might want to check up on it, but if you're not related they won't be able to find that you are. Or, if you have a really common surname they might just not worry.

You have nothing to worry about like others have said. Just go ahead and use him/her as your referee. If UCAS/a university gets suspicious, they can always do a background check and discover for themselves that you're in the clear.

Your referee will go down as your headteacher anyway so it doesn't matter what surname your English teacher has.
