As in you need to write code to do it, or you simply need to get it done?
What language are you using?
Do you understand what Hex/Dec are and how they work?
3. You might find some help on wikibooks. If you have access to the Nelson Thornes kerboodle site, that might also help.
4. Dernary is much more commonly called decimal, this is pretty good http://www.permadi.com/tutorial/numHexToDec/index.html
5. The way I would is to firstly convert to binary and then binary to decimal like below

eg. what is C3 in Denary

C=12 in denary
3=3 in denary
1100 equals 12 and 0011 equals 3 therefore 11000011 = C3 in Binary
then 128+64+2+1=195
so 195=C3 in hex
