I don't know whats worse, being rejected by a girl or being rejected for medicine
    Obviously the latter.

If your talking about being being afraid of approaching a girl, its an evolutionary characteristic.
In the past, by approaching females, we had to be aware of other competing males, which caused a fear response.
This fear response would help us fight or run if the situation of a competing male would arise.
Basically over thousands of years this was carried on , and we are naturally nervous about approaching women, even though its irrational now.

It's not entirely irrational. More people die of embarrassment each year than those who die zoo animal attacks. True story.

Physical pain and intense feelings of social rejection “hurt” in the same way, a new study conducted at Columbia University shows. The results, published in the March 28 issue of the Proceedings of the National Academy of Sciences, demonstrate that the same regions of the brain that become active in response to painful sensory experiences are activated during intense experiences of social rejection.

Embarrassing, awkward and could potentially ruin your relationship with the person.

It depends on how you feel to the girl, if you really really like her then yes it hurts but if you barely know them and you ask "do you want to get a coffee?" and they say no, you shrug and move on its no great loss to you.
