    I am in the process of filling out an application form to renew my passport. My name was changed by deedpoll when I was about 10, do I need to put my old name on the application form where it says 'Maiden/other names' even though my previous passport was in my new name anyway.
    I just want to get it right so I don't get the form sent back to me, as I am travelling soon.

    Yes I know I use my new name on the normal 'Surname' part of the form but there is a part under that, that says Maiden or previous names.
    I changed to my dads surname.
