If you're ready, you're ready, so it doesn't matter how long it will take, as long as it's right.

I'm sure your instructor will be able to inform you better.

How slowly were you going on the duel carridgeway to fail?

I would say deifnatly have 1 lesson and probably do a practice test if you feel confident. If there are areas youre not confortable with then obviously go over those but i would book it asap if i were you as there is generally a 2-4 week lag (well around here there is)

Without sounding like I am putting you down, 3s and 15m is quite alot - so you may want to have more lessons inbetween than if you had just failed with say 1s and a few minors. Only you and your instructor will be able to decide though! Are they things that you normally do or was it nerves? If you normally drive better than that then you need to concentrate on sorting it out so that you can drive well on the test day. But if that is how you 'normally' drive you will need a few lessons to iron out any problems.

The speed on the dual carriageway you should be able to sort out ok. The turn in the road observations you can work on - just do loads and loads of them. The mirrors are just something that you need to remember, which will come more with practice.

You need to balence the amount of lessons (and cost) with the amount it would cost you to fail another test - if that makes sense!! If you try and skimp on lessons, and aren't ready for it, then it will cost more in the long run to re-do your test again.

Maybe you could ask your instructor if there is someone else from the driving school (assuming that s/he is from a school) who could take you for a mock test and see how they think you are doing - its might be good to get someone elses point of view (it helped me!)

You also need to think about the minors that you got - were they for different things or an accumaulation under a few areas? Again, are they things you do normally or was it just on the test? You want to try and make sure that you don't just sort the serious things out and then fail again for an accumulation of minors under one thing.

how can you quantify it? its entirely up to the individual- for what its worth I had 2.5 months off lessons (a term at uni) and then did 4 lessons before my next test...shouldn't have failed the first time though really.

I had one before mine, the day before my test, just to get used to the car again. I failed the first with four minors and a major, and passed the second with four minors.

I didn't want anymore practice because I knew I failed on something silly and didn't think any more lessons would have helped me.
If you think you know what you did wrong and can keep it in mind for next time and pass, only have enough to keep you from going rusty. If you think you need more practice, forget about the test for now, have another lesson, and rethink when you will need to be booking a test.
^^ I was similar, except I wasn't very motivated to do another test or more lessons! I booked another test straight after I failed first time but I really was fed up of learning so only had 2 lessons before my test, which was two months after the first! Luckily I passed because I would not have continued learning, it just got so boring!
