I don't think I can answer all of your questions without giving "sex tips" but...
First, the majority of women can't orgasm from penetration alone, They need to have clitoral stimulation as well... the screaming with pleasure idea you have is really an exaggerated idea from porn and movies.
The needing to pee feeling is probably him stimulating your g spot, if he keeps doing that at the right pressure and speed you might orgasm

Yes I think ALL women lie and pretend they orgasm from it. They want to please their partners which makes every other woman feel inadequate cos we tell the truth.

I don't enjoy it at all. And afterwards you worry about stds, utis, is the guy going to stick around? It's ridiculous. Now I get why women pretend they have headaches. It's just not worth the shower afterwards let alone all the stress.
