You get them from your doctor, but usually just ask at reception.

What's a 'self ceritification form'? Sounds useful...

You can 'self-certificate' for the first 7 days you're ill i.e. you just say that you were ill without it being confirmed by your doctor.

After 7 days you can go to your GP's for a med3 - a sick certificate basically and the Dr writes what's wrong with you and for how long.

Employers should be fine with self-certification, but if not you can still ask for one from your GP - however there's usually a cost (in the surgery I work it's £10 for a private certificate).

Hope that helps,

Thanks for the info. The problem I have is that I was off ill last week with that vomitting bug thats going around, I would have gone to the doctors but I honestly felt too ill and couldnt leave the house whilst there was a risk I could throw up/need the loo at any moment. So I need something to prove I was ill, but Im not sure Id be able to back date a self certification form?
