Quick Interview by Licensed Professional

Hello,

I have been trying for some time to see a counselor for a 20 minute interview, but I keep getting cancelled on or unable to match their schedule. Duty to clients is most important of course, but it makes the completion of my homework assignment rather difficult.

Would a licensed professional (ie. LCPC, social worker, psychologist, pastoral counselor or psychiatrist) please answer the following questions for me? It would be an enormous help.
