Firstly, ring up the placement to ask whether they take students and who you should write to.
In your cover letter, explain who you are briefly (ie name, age, where you are studying, what good qualities you have in general to make it sound like you'd be a good student), why you wish to carry out work experience, and what dates you are available. Include you name and address at the top of the page, plus contact details (phone/email), and format your letter appropriately (google 'formal letter layout'). On the page overleaf, include a CV with your educational history (GCSEs, A levels, other qualifications and grades you achieved), relevant extra curriculars and interests.
If you don't get a response in a week or two, perhaps go to them in person or ring them up as you are typically on the bottom of their priority list.
Hope that helps. Quote me if you can any other questions.
