Student Loan renew loan form issue

Hello,

I would like to ask you if you could help me with my student loan form. On my Student Finance account, the form to renew my loan for the academic year 2013/2014 was sent through post on February. Unfortunately, I don't know what happened but it never reach destination so I wanted to download it to fill it in.

My problem is that on the Student Finance account, the form exist on GIF format. Downloading it means that it is saved in a photo format having 2 pages on the same side. When I crop it, the quality of the form is demaged and when I print it, it all end up in pixels. The quality of form is so weak that I'm afraid that not even my Reference Number can be read.

So, I would really appreciate if someone would give a helping hand with it and tell me how can I get my hands on my renewal loan form.
