Contact the exam board direct (OCR/AQA/Edexcel) and ask what their procedure is. Even though you were an external candidate you probably had to go through an exams officer wherever you sat it and they may be able to give you some advice. The medical note won't be any use if the date on it isn't prior to the exam unless it can be proven that the condition was present during the exam and would have caused impairment.
If you didn't mention anything to anyone (invigilator, exams officer, exams board) on or before the day then the extenuating circumstances will have to be watertight and not just a 'hayfever' excuse.
