Vaccination records

Can anyone tell me how to get hold of a "computerised vaccination history form"? I have to take it to my health screening thing in a few weeks, and although I've already sent back the form signed by my doctor with all my vacciantions written on, how am I supposed to get a computerised version?

Sorry, it's probably really obvious but I'm just not sure. Thanks!

Maybe if you ring your GP they'll be able to give you a print-out of your vaccinations? That's the only thing I can think of, hope it helps!

Didnt know you had to get a computerised form, which uni is this for? I have literally just come back from my doctors having had my 1st Hep B jab! It hurt really bad :frown: Gotta go to a health screening at Coventry on monday which is a pain cos i cant get all the things filled in till I have my other 2 hep B jabs, bit confused with that!

Why did that jab hurt, i have had all three hep's and none of them hurt at all. was it an inexperienced or very sadistic nurse who did the injection?!

It's for Manchester - I have to go for the health screening in July (including Hep B jab and blood test) and they want a computerised version, as if the handwritten and signed version my doctor's already sent to them isn't good enough.

iv never heard of this before i think your best bet is to phone them and ask them exactly what is it they want... my vaccinations are always hand written... but we do get issued with a photo ID vaccinations card which we show at interview or when applying for courses etc.

Word of warning to you all: If you are told that the hep b vaccine is egg based then they are lying...it is a yeast based injection..not helpful when im allergic to yeast and they didnt tell me this and so ended up in hospital with an anaphalactic shock!!!!!!!!

hey everyone. iv got a conditional firm for childrens nursing at northumbria uni. conditions being clear health check and criminal record check. had my criminal record check but nothing seems to be being done about my health check.

phoned them a few times and dont realy get anywhere grrrrrr

what have u all heard from your uni's about getting a health check? also, iv not been told anything about getting my hep b jags...

is it just northumbria being crap?!

I've just had a letter asking for my academic reference which needs to be returned by the 27th June, and Teesside have also said they will sending out information at the end of June. Presumably health and criminal checks etc will form part of this information.

Hi,
I'm going to Northampton to study/train as a paramedic. I also had to get a history of my vaccinations as well as getting my hep B. I was sent a form by the uni and I simply made an appointment with my GP and he gave me all the details at the surgery.

My Hep B test really hurt me as well, but I often get that trouble with jabs, so nothing new there, and I have to have 2 more of them before the end of the year. In fact the next one is due in just over a week.

I have to go for an occupational health assessment in 2 weeks time, which is the only thing worrying me now, oh and I need to have passed my driving test, and I failed it on Saturday so need to get that done too.

All looking good though concidering how hard it was to get a place on the course in the first place!

I got a letter about a week ago, asking for all my vaccination records,a health questionnaire, and consent for my medical records to be viewed by the uni. i think that once they have viewed my health questionnaire, vaccination records and medical files, they will get in touch with me regarding what vaccinations i may need and they may invite me down for an interview (i had to do one in jan for south bank) giving me a physio and asking me questions about my health.

So i presume your iuni will send you something similar soon, i think it varies across uni's. If you havent had your heps though, just book an appointment with the doctor to get it done, you don't really need to wait for the uni to tell you to do it, because you will definately have to have them done for the course.

Pre-Course Health Screenin - for manchester, July.
Health interview 30 mins approc, screening for:
Blood test,
TB,
Hep' B vacination,
& a base line long function test (never did this at nottingham)

Does anyone think that manchester are taking the piss! Nottingham was so easy, in and out. No fuss at all!
