Was never sent a declaration form but need it to get loan

I have been sent all the information about the loan I should have received at the beginning of the year. The letter said I needed to send a declaration form of but i' haven't been sent one and in the letters online it doesn't show one that I can print off. How do I go about getting my loan?

Did you apply on paper form? If so you will not need to send a declaration.

If you applied online, your declaration is the back pages of your notification of entitlement.
