Adverse possession

If a section of land isn't mentioned on the land registry title plan does that mean the title owner does not own that section of land- even if it was mistakenly left off the plan?

Also,

If the squatter explains that they know the land belongs to the true owner- can they still make a claim of adverse possession?

In the case of registered land- does the 10 years of possession continue from the previous owners?
