if you want to do optometry, then surely you'll have your reasons.
make them clear in you Personal Statement, say what interests you about the subject.
other than that write about yourself, your hobbies and interests, extra curricular activities etc.
it shoudln't take too long.
if you've done work experience mention that, maybe link it in to why you decided that optometry was for you.
