    I'm really confused about how to use the apostrophe in the following sentence, would I say:

    The Smith's house

    or

    The Smiths' house since there's more than 1 of them?

    It all depends on the context.

    If there was only one Smith living in the house it would be "Smith's house." If there were multiple people in the house it would be "The Smiths' house."

    The context is that the OP said the Smiths' house, so was clearly talking about a family of multiple people.
