Does muscle originate and insert as a tendon. I've read confusing articles on the internet. Does it originate from bone through a tendon distally and insert into a bone through tendon proximally?

Yep, your last sentence is almost right! Swap the order of "distally" and "proximally" and you've got it right! The chain goes bone -> tendon -> muscle -> tendon -> bone. The origin is more proximal; the insertion is more distal.
