If it's not real then it could be.
For example, a person could consider it a social commentary on power and dangers of its abuse. A similar thing is be A Clockwork Orange, which rather than being just an orgy of violence, is really is a story about how it is the opinion of the author that it is pointless to try to reform the worst criminals, because they only understand fear and violence.

Read about it a lot but could never bring myself to watch it, sounds pretty horrific.
I also would never watch it either. reading the plot was enough. since I discovered that movie I have trouble falling asleep. I even get scared of taking a bath
