Use the Transport for London Journey Planner

It reckons it should only take 20 mins, taking the bakerloo Line to Baker Street and then the Jubilee Line to London Bridge - easy.
Failing the suggestion above, which is by far the best.

If for any reason you find that you can't change off the Bakerloo line at Baker Street, then stay on the Bakerloo to Waterloo, and jump on the Jubilee for two stops from there to London Bridge.

The best way.

From Paddington get the Bakerloo Line to Baker Street
At Baker Street, change for the Jubillee Line to London Bridge.

Will take about 35 to 40 mis

The best route by walking distances and efficiency:

I'm guessing you'll be getting off at Paddington Overground which is closer to Bakerloo, Circle and District lines than the Hammersmith & City.

Best Way
1) Bakerloo to Baker Street
2) Change at Baker Street and walk to platform (can see it from Bakerloo platform) where the Jubilee line will be taking you southbound straight to London Bridge

Second best
1) Get the Circle line to Kings Cross station
2) Once off a KC, a short walk up and down the stairs straight in front of the Northern line (Bank Branch) which will take you to London Bridge

Don't get Hammersmith as its a bit of a walk at Paddington.

PS - The journey planner is really not as good as a Londoner who knows the system inside out. For instance the walk from Kings Cross Thameslink to the Circle line is about 10 minutes. TfL don't cater for these discrepancies. Its true for Bank / Monument, Paddington, Euston (Square), Tower Hill / Fenchurch Street etc. A lot of your journey time is taken up by walking through tunnels
