History A2 coursework record sheet

Hey everyone!
I am a bit stuck on my history coursework, seen as my teacher is not the best at her job, and i am hoping that someone will help me.
this may seem fairly stupid of me, but we have been given these record sheets by our teacher yet i have no idea what to do with them. does anyone know what we are supposed to do?
do we just list all the sources we have used to answer the assignment question?

Are these official exam-board sheets? If so, you definitely have to declare all sources you used.

They get sent off with your coursework. You're supposed to fill them out with all the sources you've looked at, even if you haven't used them for your coursework say what you used the source for briefly and then your teacher's supposed to comment on it the source you've used e.g. 'this book is good for analytical points.' And then she gives it back to you, and you add some more and follow any of her suggestions for books or what to look for in a particular book.

You just have to mention where you got your source from and put a summary of what you think about the argument
