what do the terms form, structure and language actually mean? And how would you apply them if a question asked about how a poem challenged you? thanks.

The form and structure of a poem are very similar things. Form probably = rhyme scheme etc.; structure probably = the progression of the poem within that scheme (i.e. how one stanza moves on to the next); language = the type of language used (gritty / poetic etc.)
The effects of these three features on you the reader depends on you, I guess.
