Doing revision for mocks and currently studying plants bit :/ So I learnt that water comes into the root and evaporates at the leaf but my teacher once said water enters the leaves in the xylem how can that be when water is lost there?

I think what your teacher meant is that water from the roots is transported to the leaves, and indeed throughout the plant, via xylem vessels. Upon reaching the leaves, it then evaporates back into the environment and can only be taken back by the plant via the roots.
