Due to personal circumstances I am looking to transfer University closer to home. I am just about to finish my first set of exams.

I am swapping to a course with exactly the same name, but slightly different first year modules (they have to be similar to gain the accreditation that they hold.)

Would some universities let me transfer after my first semester? Or would I at least have to finish the year?

You would have to finish the year.

Yeah, you'd have to ideally finish the year.

It's only realistically 12 weeks until it's over anyway.
