exceptional grades in foundation/intermediate

Hey i am new here and just was curious about what happens if someone got 99% in an exam but the highest grade they could have got is a B or a C, is it possible if someone got 99% or even 100% that they will be counted up a grade higher or is there no chance?

I overheard a teacher talking to a student back when I was at school about somebody taking the dual science GCSE and managing to get a B. I think you'd need to do stupidly well in both coursework and exams though.
