Wake up early in the morning, your mind is always fresher then and so you will study more efficiently. Pick a reward, i like to use food, and say that after each page of work you do you will have a bit of the food. If your exams are in a week don't bother making pretty fancy notes, just directly write out the information lots of times, even if its all scribbly and messy it will go in if you write it in your own words
Answer lots of practice questions and thats really it!
