Easy. A high plasma osmolarity is detected by the osmoreceptors in the hypothalamus. The osmoreceptors signal to the hypothalamus to signal to the posterior pituitary gland to release ADH.

ADH causes water to reabsorbed from the distal convoluted tubules and collecting ducts thus decreasing the osmolarity, maintaining homeostasis.
