yeah, i think i just used my passport. I put on my passport number and ID got cheaked through that.

hi,

im filling out my form and its asking for the serial number on my birth certificate, from what I can tell it can be one of two numbers..

theres GM followed by 6 numbers in the top right hand corner and 5 letters followed by a number in the left hand corner, there is nothing to indicate which is the serial number!

can anyone help me out and tell me which one to use?

Just wondering about this myself, as there's my NHS number at the bottom, too! On top of that, the number at the top right (which is the one i've used!) is almost camouflaged, as if it's just part of the paper.

After some debating and digging around, we've used this one.
I read somewhere that you need to '2 Letter, 6 Number alphanumeric serial number at the top'.
Remember, you need to SMALL sheet, which is your actual Birth Cert, AND your National Insurance card/a pay slip. :smile:
