I received an email saying I had received a place but also said the following...

"Please note that the offer is informal at this stage and is subject to a final check by Admissions that your qualification meets the standard requirements."

What does this mean? What is the chance of failure?

They have no proof about your qualifications, and they are aware that you might have lied about this, so they are giving themselves a get-out if you can't produce your certificates or other proof at a later stage.

If you have been entirely honest about your quals/grades, then you have nothing to be concerned about, and it *doesn't* mean your offer will be suddenly be cancelled.
