Name, address, landline or mobile is fine. Then create headings. Education, starting now, working backwards, if you have had part time employment, 'employment' or 'voluntary commitments' or Positions of responsibility' and Leisure interests' and 'references'

Divide all ur experience up under headings, ask other ppl what you have done, it is not always obvious. Write short paras about each thing, listing your role, key competencies (Google) and positive outcome.

Make sure it is neat and ask someone to check spelling

Check out Kent university careers site for examples.

    Good luck
