    I have been offered to study 4 A Levels. I would prefer to study 3 so I could dedicate more time to extracurricular activities and work experience (to acquire a more balanced profile). My worry would be that my efforts would be that my workload may be spread too thinly with the four.

    Yes 3 A levels is fine, assuming you do the right subjects.

    I'd do 3 core A-Levels as per the requirements of the med schools you're interested in applying to and one 'easier' subject only to AS level. Providing that keep on top of all your work it's fairly easy to fit in extra curric activities, work exp and voluntary work.
