Study leave letter- how do I write it? watch

    My line manager said if i want to request study leave that is longer than two weeks then it has to be "in writing", all I have so far is 'I am writing to request study leave for my A-Level exams from 7th June 2016 until the 25th june 2016.' If anyone could advise me on how to type this letter then I would be grateful.

    This is painfully formal but I imagine it should do the trick…
