Ring up the sixth form in question and ask them about the application process & requirements.

If you need a phone number you can usually find one on their website, whilst visiting their website perhaps look for information regarding the application process & requirements. It may save time and peoples patience if you don't ask for information over the phone which is readily available on a sixth forms website.

Go on their website and it should tell you. You might only have to put name email and school.
