Do you know what exactly dead beef is? I tried researching it but it was all over my head in the level of detail people were entering.

It's the correct answer. If you were a worker and a colleague on the road above you dropped a 40kg cover that happened to fall through its hole and hit you you would not be a happy (or well) man, so it is entirely relevant to why manholes should be circular.

Of course. Systems often don't allow things that happen though, which is why designs have to reduce the risk when possible.

Let me give some examples.

A fully-experienced and competent crew member was lost overboard from a yacht in the round-the-world Clipper Race yesterday, in the Pacific Ocean. She should have been attached to the boat by her lifeline, while on deck or in the cockpit, in the prevailing conditions but she wasn't.

A forklift driver died when he had a collision while driving with the forks in the raised position - a banned practice.

And, most appositely, a man working down a manhole was hit by a falling non-circular manhole cover when its tether chain broke:

He later died. It shows the value of a design that makes a fall through the cover's own hole impossible.
