Just think of basic tips and how to avoid them: i.e in chemistry you could write something like
Water may be spilled on the floor which could could a risk of slipping. This is a low risk but it can be minimised by cleaning up the spillage as soon as it happens and alerting the teacher straight away. If somebody does end up slipping, they should seek medical attention.

    Basically:
    Risk
    Hazard
    Low/High/Moderate ect
What to do if it happens
    Emergency procedure
