HC1W (Help With Health Costs) Form help! (Sending evidence) Watch

    Hello,
    I'm currently filling out a HC1W form in claiming for help with health costs (eye tests, etc).

    It asks for evidence of my student loan, but I am unsure exactly what it wants. It says: 'send us the financial assessment or support notification from Student Support Direct... it must be this... we cannot accept the schedule of payments'.

    So where would I get that information from? Would it have been sent along with the schedule of payments- if so which page?!

    I also have a Welsh Government Learning grant. Does this count as a "Maintenance grant"? An award notice is also requested for that, so what do I need for that?!


    Presumably it costs money to phone them up and ask them, so I am hoping that other people on here have sent this form and as such can help.

    Any help is much appreciated.

    Your entitlement letter should do it for both you would have got it before you started uni. It would be around page 3/4 I think.

    Your student finance breakdown will be it. No I was going to but you can't apply with savings.

    That reminds me of another question I have: the form asks whether I have any savings, below that is "Money in accounts". I have money stored in a current account, so does this have to be declared? I have only a few hundred in there.

    Yes, you have to be as honest and as accurate as possible. The student finance breakdown should be on a letter they sent you before the start of the academic year detailing how much you will be getting. Send photocopies of this because you won't get it back but if you're claiming money back as well as filling a HC1 form, then the receipts have to be original.
