Could an AS level in creative writing taken in year 11 that came out as a B or C jeopardise an offer for medicine at any university if the four a levels to be taken in year 13 are predicted A*s?
Would it even have to be declared?

I doubt it, but could depend on the university. My friend did critical thinking in year 11, getting a C, and got into med school fine, and several friends (including myself) took critical thinking in lower sixth as a 5th AS, and didn't make any difference

Most universities don't count it whether it's good or bad. So you'll be fine.
