For dentistry, you can only pick four universities so my fifth choice must be a different course. Can I pick economics as my fifth course? Or is it too unrelated?

You could, although you would almost certainly be rejected. Perhaps pick a course related to dentistry at a university where it is quite easy to change degrees after enrolment.

I would echo the above sentiment.
More importantly, however, is why do you want to study economics? If you are interested in dentistry, surely something more biomedical related as a backup would make more sense?
