I didn't do very well on my GCSE's which wasn't a concern at the time but now that I have decided I want to do medicine I was thinking maybe after college I could do some extra GCSE's to compensate for this.

    My AS projected grades are:
    Biology - A
Chemistry - B I think I shall get an A but we will have to see
    Physics - A
    Maths - A (I'm dropping this)

    For GCSE I got:

    Biology - A
    Chemistry - A
    RE - A
    Physics B
    Additional Science - A
    English Language- B
    English Lit - B
    German - B
    History - B
    Maths- B
    Geography - C
    Business - D

Is it really unrealistic for me to do medicine???

You might be alright with the GCSE grades you have. You'll have to apply smartly and check every uni's entry requirements. Some state a required number of As.

Do you think any Uni's will allow me to take extra GCSE's next year? (I'm taking a gap year)

I'm not sure. I've heard of people taking extra GCSEs but tbh they are a small part of the application for most. It might not be worth it. Maybe try and apply and if you don't get in then do the extra GCSEs in your gap year.
