There's a small risk there (couldnt give you the numbers though), the withdrawal method isnt a proper form of contraception, if its not too late maybe you should consider emergency contraception or if it is I wouldnt panic too much but take a test when its been a couple of weeks if it stops you panicking.
Emergency contraception- go speak to your chemists ASAP.
There was a recent article on the Telegraph on this. I think it is like a 27% chance IIRC. I would suggest speaking to your local pharmacist or GP for emergency contraception though.
Just use a condom.
If you don't use a condom or birth control theres always a chance.

There is no 100% foolproof method. Even people who have been told they are infertile or are going through menopause have gotten pregnant. It's part of basic sexual maturity to know that if you have sex you could conceive.
Pulling out is just a mental idea anyway, there is such a thing as pre-ejaculation. He will have leaked sperm inside of you during intercourse whether he pulled out last minute or not.

Depends where you are in your period. During and the week after menstruation you'll be safe. Any other time and I wouldn't without some reliable kind of contraception in place.

If you took Plan B or Morning After Pill under 72 hours then no need to worry.
