Yes you can. Some universities want you to meet their A Level requirement and some will just want your uni grades. I'm personally transferring to East Anglia, I don't meet their A level grades but they instead asked for 65% in my first year of uni which I got and now my offer is unconditional.

So my advice here is to ask if the university will consider your university grades alone (unless of course you do meet their A-level requirement - or any other high school qualifications you did, then you've got no worry). I'm not sure if it's too late now though since the UCAS deadline has passed.

The answer: it depends. Some will accept a transfer into second year providing that your first year is similar enough to theres. If it isn't similar enough then they may ask you to do first year again at their uni. As Captivated said entry requirements will vary- some will still want you to meet the A-Level requirements, others will overlook that if you have good 1st year marks.
