If I spoke about comparative advantage in both the case study and the essay is it possible the examiner may think I'm just a lazy kid who didn't bother to revise anything else?
Look at the June 2012 paper for example, in one of the case studies, the question is asking about whether its good if turkey joins the EU, then in the essay section it asks about the costs/benefits of free trade. Now, any person with a hint of experience of ECON4 would know that you can write a **** load of comparative advantage(at least a side and a half) on both essays. Could I do this in the exam then?

I haven't seen the paper but if it is valid in both questions yes. The issue wouldn't be whether or not you mention comparative advantage but how well developed it is.
