    - Want to do work experience at local GP
    - Haven't gotten GCSE results yet
    - Have done work experience prior at school (didn't write a CV)

What to put on CV? Literally no idea?
If you could send me a CV template with dummy text or so that would be nice

Have a look online, there are hundreds of templates out there that you can use. If you want to do work experience, maybe go in an speak to them in person though? Explain what the situation is and see if they can help. (Though, I'm not sure if GPs even accept work experience applicants, I seem to remember when I was applying you couldn't because of patient confidentiality. It might have changed, but it would be a good idea to talk to them in person first, because they might just turn you down right off and it'll save you having to write a CV.)
On the question about the CV, put down anything and everything you have done. Did you have a buddy scheme for year 7's at your school? did you help out? put that down. Were you a prefect? put that down. Afterschool clubs? put that down. They want to see that you are willing to go out of your way.
