Did anyones doctor or GP charge for filling in nursing health form?

All my friends and other student did not get charged for doctors giving signature but one of my friend was charged £25. My GP wants to charge me £70 and they said unless the full amount is paid they wont sign it.

Did anyone else have this problem? Please help because I cant exactly pay £70
