    Hello,

    I am going on a preparatory course to study medicine in Poland and the course asked me for a health certificate. No one knew what they specifically mean and my GP asked me to contact them to ask what info they require. The only thing I got back was:
    "the certificate should contain information about the absence of contraindications to study abroad." I am just afraid that the second time I go to my GP they will send me away as this is not specific enough. Maybe someone knows info I require?

    Vaccination history, including chicken pox history
    Any illnesses that you're currently undergoing treatment for, or any major illnesses in the past (asthma, depression etc).
    Any blood borne diseases, or lack of (Hep B, HIV etc)
    List of medications.
    Anything that may impact your ability to undertake a medicine degree.

    I'm going off of what is usually required by occupational health in this country. That should cover everything though.

    That's strange because similar is often required in this country too. I guess the word 'certificate' made it sound very formal and confused the GP.

    They just want a print out of your medical history to make sure you don't have TB basically.

    Be aware that your GP may charge for this, as it's not an NHS service.
