    Background: I failed 2nd year twice at different unis for mental health reasons and I've now applied for a different course at my first uni through clearing. I was interviewed and I explained that I'm ready to get back to uni now that I'm taking steps to deal with my mental health. After the interview, I've now been offered a place but I need to fill in the UCAS RPA form AND obtain a "satisfactory" academic reference from my former department from the same uni.

    I have three queries:

    1) What are they looking for in a reference? Should I contact my previous department, or my tutor?

    2) In the disability section, what shall I put in? My GP referred me to the counseling service but I haven't had my first assessment yet (it's in a few weeks), so I haven't been officially diagnosed with anything yet but it seems like social anxiety disorder with mild depression in terms of symptoms. Should I write "social anxiety disorder - not yet diagnosed" or leave it as blank?

    3) Just in case the above application doesn't work out, I'm thinking of transferring my first year credits towards an Open University course. I've filled in the credits transfer application as well, but the form says OU may contact my previous institutions for info. But if they contacted the uni I've applied to already, would it look bad to the uni? Will the cancel my application on the basis that I'm looking around for back-up options?

    1) See https://www.ucas.com/advisers/refere...ate-references the section on "What to include" - although this isn't a UCAS reference those rules still apply. COntact whoever you think will be most cooperative.

    2) Just put "Social Anxiety and Depression" for now

    3) OU would contact your previous universities THAT YOU HAVE ATTENDED about your previous attendance. It shouldn't matter about another application.
